Description

Preble Street Health Services is seeking a Medication Assisted Treatment - Prescription Drug and Opioid Addiction (MAT- PDOA) Caseworker to help address the opioid crisis in our community by offering support to individuals struggling with Opioid Use Disorder and co-occurring diagnoses through an innovative Medication Assisted Recovery Program, in partnership with Greater Portland Health. The goal of the caseworker is to provide compassionate, and respectful services supporting substance use disorder (SUD) recovery goals along with long term housing sustainability goals.

The Caseworker is responsible for supporting clients enrolled in the Greater Portland Health MAT-PDOA program. This program bridges a service gap between patients engaging in the daily observed MAT (Medication Assisted Treatment) dosing and support model and clients further along in their recovery who are accessing the GPH Greater Gateways Program for weekly appointment-based treatment and support. The Caseworker will use a variety of skills, including Motivational Interviewing, Trauma Informed Care and Strengths Based Support to help the client achieve success with their recovery and housing goals.

The schedule for this full-time (40 hours/week), 1st shift position is Monday-Friday, 8am-4:30pm.

Qualifications

Qualifications include a bachelor's degree in a social work- or mental health-related field, or a bachelor's degree in an unrelated field and one year of full-time, relevant experience. The Health Services Caseworker must meet the qualifications to provide Targeted Case Management (TCM).

Additional requirements include positive regard for individuals who are homeless and struggle with substance use disorders; experience with harm reduction and low-barrier approaches to working with this population; effective outreach and engagement skills; and ability to work as part of a team and with collaborative partners from both medical and social services organizations.
