The Catholic Charities Maine Asylum Seeker Transitional Housing Program is now hiring for a Full-Time Health Case Manager for Asylum Seekers. This position will be split between our South Portland and Saco locations.
The Asylum Seeker Transitional Housing Program is a new program designed to provide support to asylum seekers living at hotels in Saco & South Portland, under a 12-month contract with the City of Portland. Services will be delivered in a culturally and linguistically appropriate manner and include on-site support, case management, assistance connecting with/enrolling in community and public resources including education, health care, legal and employment services.
Definition: The Health Case Manager for Asylum Seekers, at the Asylum Seeker Transitional Housing Program (ASTHP), is the person who will provide access to health resources to asylum-seeking individuals & families who are temporarily housed. This position is part of a dynamic team providing support to adults and children seeking asylum in the U.S. The Health Case Manager for Asylum Seekers will be a trained, supervised professional responsible for assessing the unmet medical and physical needs of the asylum-seeking clients in the program and making applicable referrals for client care. The person in this position will coordinate and manage physical health services as well as mental health services, when applicable.
